Our physical therapist-written and researched parameters of care are grouped by anatomical region and cross referenced by diagnosis. Each guideline provides appropriate evaluation tests and measures, functional goals, appropriate therapeutic intervention, discharge criteria, and considerations for additional visits. CareConnections Therapy Referral Handbook
An excellent tool to provide to referring doctors, our handbook lists “Considerations for Referral” by anatomical region, which is followed by a two-page summary of each condition’s Clinical Practice Guideline. The Therapy Referral Handbook’s cover can be customized with your clinic’s logo to be used as a marketing tool to enhance your physical therapy referrals and reimbursement.

CareConnections Outcomes System
A functionally-based patient data collection system, our user-friendly online system compares clinical outcomes to similar outpatient physical therapy clinics and physical therapist-submitted episodes of care. Outcomes reports provide valuable clinical information and management data that can be used to promote best practices, improve quality of care, and add value for those responsible for the costs of care. CareConnections Patient Satisfaction Monitoring
As an additional component to our Outcomes System, the Patient Satisfaction System provides valuable feedback about a patient’s physical therapy and provider experience. CareConnections Prior Authorization/Advice System (PAS)
PAS ties all of the above together into one component that allows 24/7 Web-based dissemination and acquisition of utilization management information. PAS delivers prospective utilization of care in the form of an appropriate number of visits per specific patient condition along with an appropriate CareConnections Clinical Practice Guideline. PAS provides concurrent utilization management based upon initial measures and continued monitoring using the CareConnections Outcomes System 
and provider requests for additional treatment. It also provides retrospective utilization management in the form of CareConnections Outcome reports and benchmarked utilization statistics. For more information on PAS, click here.
