Equal access to physical therapy for all individuals.

We are preferred providers for most insurance plans and will gladly bill them directly. We also offer payment options for individuals who do not have health insurance coverage, have exhausted their insurance benefits, or who choose to not use their health insurance. We believe everyone should have equal access to quality physical therapy.

Our insurance verification process:

Whether you are a first-time patient of physical therapy or a loyal, returning customer, it is our goal that you feel confident in understanding your insurance coverage.

When you call to schedule your first appointment, we will ask you for your insurance information. Then, as a courtesy to you, we will call your insurance company directly to verify your benefit coverage and will review this information with you before you begin your first visit.

We also encourage you to call your insurance carrier to find out the physical therapy benefits provided by your medical plan prior to your visit. Here are some questions you may ask your insurance:

Does my health plan require a referral and/or an authorization for outpatient physical therapy?

What is my deductible and out-of-pocket per calendar/plan year? How much of that has been satisfied to date?

Is this benefit subject to a co-pay at each visit or a coinsurance?

Our insurance billing process:

Your appointment time has been reserved just for you. Keeping all appointments is an integral part of your recovery. If you should need to cancel an appointment, please call the clinic as soon as you know, so that you can reschedule your visit. We require 24 hour notice for the cancellation of a scheduled appointment.

Is a physician’s referral needed for insurance to pay?

State laws allow consumers to access physical therapy services directly, without the need of a physician’s referral. Insurance coverage and the requirement of a physician’s referral, however, is plan-specific. Most insurances do not require a referral, but some do.

If you do not have a physician’s referral for physical therapy and are uncertain if one is required in order for insurance to pay, we recommend that you contact your insurance company directly to verify this. We are also happy to verify this for you prior to scheduling your first appointment.

Billing made easy for you.

Co-payments are due at the time of service. We gladly accept cash, check, or credit card (Visa and MasterCard) at all of our clinic locations. Once insurance has processed your claims, if there is a remaining patient responsibility (deductible, co-insurance, etc), we will mail you an account statement that reflects the current balance of your account at the time the statement is processed. Should you have any questions regarding your statement, please contact our customer service department directly (800-219-8835 option #1). They are available Monday thru Friday from 8 am – 5 pm.

Self-pay physical therapy:

We want to ensure that all individuals have access to quality physical therapy care. We offer affordable rates for patients without insurance or who wish to not use their insurance. For self-pay accounts, payment must be made at the time of service. Some restrictions may apply with patients who qualify for Medicare, Medicaid, or other specific insurance plans.
