Our physical and occupational therapists are professionals dedicated to educating, training, and applying principles of tested medicine and knowledge of the healing process to serve each patient’s unique condition and goals for recovery. Referral pads
You have many options when making a referral to the clinic most convenient for your patients. Our preference, however, is to receive a fax from your office directly to the clinic. You can find the fax number to each clinic on the regional referral pads. This process will help ensure that your patients get the care your physicians recommend, and we will do everything we can to contact the patient as quickly as possible to schedule their initial evaluation at their convenience. For your convenience, if your physicians need additional referral pads, simply contact the location nearest you or submit a request via email.

Making a Referral?
