David Dowd - OMG Orthopedic & Sports Medicine

David Dowd, PT, DPT

Managing Director | Physical Therapist

David earned his BS in Biology from State University of New York at Cortland in 1993. He spent the next 6 years operating a personal training business and still stays active in the field. To supplement the knowledge of exercise, David received his Masters of Science in Physical Therapy from California State University at Long Beach and has specialized in orthopedic physical therapy for the past 7 years. His specialty interests include resistance training, post surgery, sports rehab, and manual therapy. In his personal time, David enjoys hiking, backpacking, canoeing, music, and spending time with his wife and two daughters.

Adam Fagan - OMG Country Club Road

Adam is a native Oregonian who graduated from the University of Oregon with a Bachelor of Science degree in 2007. After working as an Aide for TAI in Lake Oswego, he moved to Chicago to complete his Doctorate of Physical Therapy at the University of Illinois, at Chicago. Upon completion at UIC, Adam returned to Eugene to join TAI at Country Club Road.

Adam is passionate about treating a wide variety of patients, with special interests in utilizing manual therapy to treat orthopaedic and sports-related injuries. He plans to continue advanced training in these areas.

Adam was lucky enough to get to marry the love of his life in July of 2016. When Adam is not working you can find him outside with his family enjoying hunting, fishing, camping, or competing in archery.


Kristen Allen, PT, DPT

Kristen is an Oregon native who attended Corban University in Salem, Oregon for her undergraduate degree and completed her Doctorate of Physical Therapy at Pacific University in Hillsboro, Oregon. She, like many other physical therapists, discovered her interest for physical therapy while rehabbing a shoulder injury in college. She enjoys treating a wide population of patients, with a special interest in using manual therapy to treat orthopedic injuries. Kristen is passionate about getting people moving, active and back to doing things they want to be doing and is continuing her education through the North American Institute of Orthopedic Manual Therapy.

In her free time, Kristen loves living in Oregon and enjoys all the outdoor adventures it has to offer, especially hiking and trail running. She also enjoys Crossfit, drinking delicious coffee, reading and traveling to cool new places.

Jeff Bonezzi - Therapeutic Associates Physical Therapy at OMG Country Club Rd

Jeffrey Bonezzi, PT, DPT

Physical Therapist

Jeff received his Doctorate of Physical Therapy from Chatham University in Pittsburgh, Pennsylvania. Clinically, he has focused on outpatient orthopedic settings managing complex musculoskeletal disorders, working with people of all different backgrounds. Jeff has a strong belief in the use of manual therapy within physical therapy practice and has special interests in sports/ACL rehabilitation, concussion management, and chronic pain. He also performs running evaluations to fine tune gait mechanics and prevent injury. As a USATF level I coach, he has years of experience coaching runners of all ages. Prior to physical therapy school, Jeff earned honors in a Bachelor of Arts in Physical Education and a minor in Physics from Baldwin Wallace University while he participated in soccer, cross country and track. Born and raised in Northeast Ohio, he decided to move across the country to TrackTown USA to practice physical therapy in Eugene, Oregon. When not at work, Jeff can usually be found exploring new running trails, hiking national parks, and cheering on Cleveland sports teams, Go Tribe!

Samuel Cornell - OMG Country Club Road

Samuel Cornell, PT, MSc

Samuel was born and raised in Ontario, Canada. Prior to calling Eugene home, he lived and worked in Vancouver, British Columbia and Honolulu, Hawaii. An avid sports fan, Samuel grew up playing baseball, soccer, and rugby. He gained his interest in physical therapy after rehabilitating a shoulder injury he sustained playing rugby. After earning his Honours BA in Kinesiology from the University of Western Ontario, he went on to complete his Master of Science in Physical Therapy from the University of Toronto. He has been practicing physical therapy since 2010. Samuel enjoys treating all orthopaedic injuries and has a special affinity for ankle, shoulder, and knee injuries.

In addition to working full-time, Samuel is currently in his second year of an orthopaedic fellowship program through The North American Institute of Orthopaedic Manual Therapy. In his spare time, he enjoys working out, hiking, board games, playing soccer, and cheering on the Toronto Blue Jays.


Tia Dankberg, PT, DPT

Physical Therapist

Tia, a native Arizonian, completed her B.S. in Family Studies and Human Development at the University of Arizona. While attending U of A, she worked as an athletic training technician with Arizona Athletics for 2.5 years, providing medical support and game day prep for multiple sports including football and track and field. She earned her Doctorate of Physical Therapy from Midwestern University in Glendale, Arizona. While in graduate school, Tia had the opportunity to provide physical therapy services to rural communities in Guatemala for a week, two consecutive years. She held various leadership roles and participated in multiple community service opportunities earning her the Award for Outstanding Community Service at Midwestern. Tia’s clinical interests include pelvic health, orthopedic rehabilitation, and sports medicine.

In Tia’s free time, you will find her wandering a bookstore, drinking coffee or exploring various hiking trails and scenic views with her camera in hand. Though she lives in Eugene, Tia’s heart will still belong to the Arizona Wildcats and all Arizona sports teams.

Abigail Dowd

Abigail Dowd, PT

Abigail grew up along the West Coast of the U.S. during her younger years but returned to her homeland, where she completed and earned her Physical Therapy degree with distinction from Angeles University in the Philippines in 2002. She relocated to the Pacific Northwest to pursue her career and has continued her education with advanced training in the North American Institute of Orthopaedic Manual Therapy and the Herman & Wallace Pelvic Rehab Institute.

Over the years, Abigail has worked with a variety of musculoskeletal disorders of both the spine and extremities in the young and aging population. Her main focus now is working with the pelvic floor population. She has extensive advanced training and clinical experience working with all patients for urogynecological, obstetric, and menopausal conditions (urinary/fecal incontinence, biofeedback, pelvic floor training, pelvic floor pain management, pregnancy-related musculoskeletal dysfunction, pre/post-natal rehab, and colorectal conditions) as a pelvic floor specialist since 2006. Her commitment to her patients is reflected in dedicated, one-on-one treatment, with emphasis on her patient’s goals, reinforced by the latest research and results.

Outside of being a Physical Therapist, she enjoys traveling and spending time with her husband and two daughters. Abigail also is enthused about fitness, outdoor running, cooking and creating healthy recipes, gardening, and volunteering at local schools.

David Holte

David earned his Bachelors degrees in Biology from Wartburg College in Waverly, Iowa where he also played soccer all 4 years. He obtained is Doctorate in Physical Therapy from the University of Iowa while also becoming a Certified Strength and Conditioning Specialist (CSCS).

David enjoys working will all variety of patients with various conditions to achieve their goals using individualized treatment. He plans on pursuing further certification in manual therapy with a goal of becoming a Fellow in Manual Therapy. His special interests include concussion, exercise prescription, vertigo, running, manual therapy, orthopedic, and sports rehabilitation.

When he is not working he is coaching for Eugene Timbers FC, cooking, running, hiking and other outdoor activities.

Lindsay Lund PT, DPT

Lindsay Lund, PT, DPT

Physical Therapist

Lindsay received her undergraduate degree in biology from DePauw University and her Doctor of Physical Therapy from the University of Colorado. She developed a strong interest in physical therapy while rehabilitating a knee injury sustained during collegiate soccer.
Upon completing her physical therapy degree she stayed in Colorado to work in an outpatient orthopedic clinic where she served a diverse patient population. She continues to enjoy treating an array of conditions, but has a special interest in treating active individuals across the lifespan. Lindsay enjoys learning about her patient’s lifestyle and goals to better guide her when constructing an individualized plan of care. She also enjoys seeking out opportunities to expand her knowledge and skills in order to improve the care she provides.

Outside the clinic, Lindsay enjoys exploring the outdoors through running, mountain biking, skiing, rock climbing, and more.

Dan White - Oregon Medical Group - Therapeutic Associates Physical Therapy

Daniel White, PT, DPT

Physical Therapist

Dan is a native Iowan who attended the University of Iowa. His undergraduate major was Exercise Science and he then received his Doctorate of Physical Therapy in Iowa City. Dan spent his first 11 years practicing in his hometown of Bettendorf, IA. During that time he built a reputation treating local runners in a large sports medicine clinic. He also led his companies first year training program for 31 clinics and was also on adjunct faculty at the University of Iowa.

For the past 5 years Dan has partnered with David McHenry of Therapeutic Associates’ PACE network and the Nike Oregon Project to expand his treatment of runners and track athletes. In 2017 he joined PACE and Therapeutic Associates in Eugene. He is the physical therapist for the University of Oregon Men’s and Women’s track and field and cross country teams. He counts himself lucky to work at Hayward Field and at Country Club Road. He strives to provide Olympic level care to his Ducks and all the runners of Track Town, USA.

Dan’s area of expertise includes sports medicine, endurance sports, gait analysis and correction, ACL rehab and prevention, and he is board certified in orthopedic physical therapy. He is an RRCA certified running coach as well.

In his free time, Dan enjoys running the trails and roads of Eugene and is exploring all that Eugene has to offer, including the breweries. He is married to his exceptional wife, Kelly and has three amazing children; Lucas, Lauren, and Lydia. There is never a dull moment with those three kids.


Maddie Williams, PT, DPT

Maddie Williams is a native of South Carolina where she received a Bachelor of Science degree in Pre-Physical Therapy from Bob Jones University in Greenville. Maddie received her Doctor of Physical Therapy degree from the Medical University of South Carolina in Charleston. During graduate school, Maddie was involved in student leadership within the American Physical Therapy Association, and she continues to be an active member in the association today.

Her clinical interests include chronic pain, orthopedics, and manual therapy.

In her free time, Maddie enjoys staying active through weightlifting, soccer, hiking, camping, and exploring her new Oregon home with her dogs.

Donald Hooley - OMG Country Club

Donald Hooley is a native of New York who recently moved to the Eugene area. Donald received his Bachelors and Doctor of Physical Therapy degree from Ithaca College in upstate New York. After school, Donald was selected into Cayuga Medical Center’s Orthopedic Residency program to further his education with a specific focus on manual therapy techniques and psychosomatic effects of chronic pain.

Donald’s clinical interests include patient education and psychological impacts of chronic pain, low back and neck pain, and a wide array of orthopedic conditions.

In his free time, Donald enjoys fly fishing, hiking, exploring his new home with his girl friend and being a cat dad of two.

Carrie Meadowcroft, OTR/L

Carrie is a Michigan native who moved to California to complete her degree in Occupational Therapy at Loma Linda University.
She received her BS from the School of Allied Health Professions in 1992. She has 25 years of experience in Upper Extremity and Hand Therapy in both California and Oregon, and has credentials from the Hand Therapy Certification Commission as a CHT (Certified Hand Therapist).
In addition, she is certified to use the Graston (R) instruments for assisted soft tissue mobilization, and has a certificate in Kinesiotaping.
Her further accomplishments are in the arena of work ergonomics, and giving educational talks in a variety of settings on ergonomics, gardening, living smarter with arthritis, and common hand injuries.

Topics of special interest are custom splinting, CMCJ (Thumb) arthritis, controlling scar formation, and watching wounds heal.
She believes the best thing about being a therapist is helping people return to using their hands for the things they love and need to do.

When she is not enjoying her patients, she is running, hiking – and most recently backpacking, (especially to discover waterfalls), keeping track of her wonderful dauthters, leading a worship team with her husband and friends, or curling up with a good book.
