Angela Lewis - TAI Physical Therapy at Crescent Village

Angela Lewis

Clinic Director | Physical Therapist

Angela graduated Cum Laude from Oregon State University in 2000 with a degree in Exercise and Sports Science. In that same year she earned her Athletic Training Certification through NATABOC, and then went on to attend Pacific University where she graduated with distinction and earned her Doctorate of Physical Therapy in 2004.

Angela is a board certified Orthopedic Clinical Specialist with a focus in manual therapy and return to basic and high level function. She is currently pursuing advanced manual therapy certification through the North American Institute of Orthopedic Manual Therapy. She also has advanced training in The Astym™ System and Fascial Distortion Model. Angela’s holistic treatment philosophy emphasizes the overall wellbeing of her patients. Her commitment to her patients is reflected in dedicated one-on-one treatment, with emphasis on her patient’s goals, and reinforced by the latest research and results.

Outside of being a Physical Therapist, she enjoys traveling and spending time with her husband, daughter, and golden retriever. She loves to spend her time outdoors skiing, running, hiking, and camping with friends and family.


Sam Stewart

Physical Therapist

Sam graduated Magna Cum Laude from Western Oregon University with a Bachelor’s of Science Degree in Exercise Science. He later moved to Flagstaff, Arizona where he earned his Doctorate of Physical Therapy Degree from Northern Arizona University.

Sam enjoys treating patients of all ages with a wide variety of conditions and skill levels. He aims to provide his patients with the care needed to optimize their overall health and wellness through research and experience. His clinical strategy often includes a manual therapy approach in combination with patient specific exercise prescription to improve an individual’s function in an efficient and holistic manner. Sam is currently pursuing advanced clinical education from the North American Institute of Orthopedic Manual Therapy (NAIOMT). Additionally, he hopes to become a Certified Strength and Conditioning Specialist (CSCS) through the National Strength and Conditioning Association.

A native Oregonian, Sam grew up in Pendleton, where he was heavily involved with both school leadership opportunities and athletics, competing in football, baseball and wrestling. Due to his lifelong involvement in sports, he spends most of his free time exercising, hiking, and participating in community athletics. Additionally, Sam enjoys traveling with his girlfriend, Marcella, and spending time with his family and friends. Raised in a family of lifelong Oregon Duck fans, he looks forward to treating the great people of the Eugene and Springfield area.

Lizzy Treeheart - Crescent Village

Lizzy Treeheart

Physical Therapist

Lizzy graduated from Purdue University with a Bachelor’s of Science Degree in Athletic Training in 1991. She went on to attend graduate school at the University of Indianapolis where she earned a Master’s in Physical Therapy in 1994. With her 23 years of experience, she brings a comprehensive understanding of client care from a variety of settings. Her favorite setting is outpatient.

Lizzy has specialized training in osteopathic techniques, which are gentle, hands-on approaches that assess a person as a whole and addresses the root causes of pain. These include Visceral Manipulation through the Barral Institute and Craniosacral Therapy through the Upledger Institute. These skills are effective with a wide range of clients from high level athletes dealing with injuries to others dealing with chronic pain. Her philosophy is holistic and incorporates the emotional/mental aspect by incorporating intuitive counseling when appropriate. She is loves learning and continues to incorporate new techniques to better help clients get better quicker.

She lives in Eugene with her wife, Louann and their 2 dogs. They enjoy hiking, mountain biking, camping, and kayaking/canoeing. She also volunteers time working for a non-profit organization, TheInnerRevolution.Org, fighting to meet the needs of people and the Earth. 

Michael Winn

Licensed Massage Therapist

Michael graduated from the Massage Therapy program at Lane Community College in 2015, and as Licensed Massage Therapist and dedicated amateur athlete Michael has been involved in the health and fitness movement for over 25 years. Michael is excited to join the Dreamclinic team at Therapeutic Associates to bring the best in bodywork and massage to the Eugene community. When not in the clinic you can find Michael on the bike trails, golf courses, ski slopes and single tracks all over the Pacific Northwest.

Kyla Coy

Licensed Massage Therapist

Kyla graduated from the New Mexico School of Natural Therapeutics in Albuquerque in 2015. Her approach is connection-based and client-centered, which means she does her best to connect to each client’s body as well as listen to their words to execute an effective treatment. When not practicing massage you may find Kyla writing, on a mountain top somewhere, or on my yoga mat.

Administrative Supervisor

Toni Maldonado has been the Office Manager of our clinic since its opening in 2014. Prior to that, she worked at Therapeutic Associates for 5 years. Toni relocated from her hometown of Pocatello, Idaho in 2005 and has loved living in the “Home of the Ducks” ever since! Her love for helping others and getting them back to a healthy lifestyle has provided a perfect fit for her here at Therapeutic Associates at Crescent Village. She goes above and beyond to assist patients on their road to recovery.

Many would describe her as a loving, kind, and warm person who gets along with everyone. You can often find her hard at work, but always with a smile on her face, as she loves what she gets to do! In her spare time, you can find her outside with her husband and four children, either rooting them on at sporting events or out on a hike enjoying the beautiful scenery around Oregon.
